eLabFTW Team Application

Name of applicant(Required)
This should either be the surname of the group leader, or the name of a module, or project involving several people. We do not create teams for individuals.
Name of team admin(Required)
Every team needs at least one administrator. Existing admins can appoint additional admins.
Consent(Required)
The personal data collected via this form (i.e. first names, last names and email addresses) will be accessible to the system administrators of the local eLabFTW instance at JGU’s data center. This data will only be used for the purpose of creating a team within this local instance. The data will not be shared with third parties.
In addition, the central privacy policy of JGU, as set out on its website at https://www.uni-mainz.de/datenschutz/ applies.
